Please print and complete the above forms and return to the Atlantic Beach City Hall Cashier's counter with your payment. Download a free Adobe Reader program here.
Annual usage fee $50 for the first dog, $25 for each additional dog. Lost card replacement fee is $15. (Sales tax will be added to all fees.)

Shot records must be completed and signed by a veterinarian before access cards and dog tags will be issued.
